On Tuesday, February 18th, authorities revealed the identity of the man who passed away in an industrial accident at Royal Oaks Monterey Mushrooms facility, California. He is 62-year-old Jose Fierros, of Watsonville.

Fierros died last Thursday, February 13th, after suffering an accident at the processing plant at 777 Maher Road. He was caught in between a piece of machinery that later got identified as a hopper.

The deceased was found in the hopper by his colleagues, and their resuscitation efforts proved abortive. Fierros passed away at about 3:15 p.m., according to the Monterey County Sheriff’s Office.

State officials and local authorities are working together to figure out what went wrong. The Occupational Safety and Health Administration is also investigating the accident, and the administrators of the processing facility are cooperating with the investigators.

Accidents often happen at processing facilities, and mostly end in fatalities. Monitoring bodies have advised workers to put safety first while working to prevent accidents.

One of the ways to ensure safety includes reading the manual of types of machinery before using them. Ensure that every machine and tool is used for the right purpose.

Workers should inform their supervisors once they become aware of any unsafe machine or tool. They should also have a helmet on always, and wear other protective clothing.

Workers should wear walkable shoes to prevent tripping and falling, and floors should be kept dry and free of potholes. Hazardous materials should be avoided, as well as falling objects, and all emergency exits should be kept open and accessible at all times.
