The Port of Houston Authority is investigating an accident that resulted in the death of 40-year-old John Tyler last Friday.
Tyler, a construction worker, was on the job at Bayport Container Terminal when he died.
Based on preliminary reports, Tyler was operating a truck that offloads containers from ships. The container did not detach fully from the crane that was transporting it and the truck lifted into the air.
The International Longshoremen’s Association, of which Tyler was a member, took to social media to discuss the issue on Saturday.
They said Tyler was offloading a container ship called the Maersk Detroit. The container was reportedly still attached to the truck when the crane took both the container and the truck about 40 feet into the air.
Witnesses said the truck crashed to the ground with John Tyler on it. He died at the scene.
The International Longshoremen’s Association Safety Director, Bob Fiore, is also investigating the incident along with Charles Montgomery, the International Longshoreman’s Association Safety Representative.
The International Longshoremen’s Association worked with the Houston chapter and sent grief counselors to speak to members.
The association’s president called the accident was senseless and preventable.
Members are hoping that the investigation will help to prevent similar accidents.
Video of the incident was released by news outlets. It showed the damage the truck sustained from the fall. The windows had broken out of it and there was debris scattered around the vehicle.
